GED Forms

 

These forms are to be submitted on behalf of 16-year-old or 17-year-old applicants who qualify for an exception to take the GED exams before the age of 18.  Further information can be found on our GED Eligibility Page.

Instructions:

  1. Applicant must initiate registration process at www.ged.com.
  2. Applicant will receive an alert message with a link to this forms page.
  3. Applicant is responsible for directing the local public school, parent/guardian or institution/court to the required forms.
  4. Required forms must be submitted by the appropriate party as stated below, not the test-taker.
  5. Upon receipt and verification of all required forms, applicant can log back in to www.ged.com to proceed with the registration process and schedule the exams.

17 Year Old Applicants:

* High School Non-Enrollment Form for 17-Year-Old Applicant (PDF)

  • This document is to be completed and submitted by the local public high school, not the test-taker.
  • Submit this form to verify that the test-taker is not currently, or never has been enrolled in your high school (home schooled, private schooled, recently moved to area).
    Faxed copies of these documents will not be accepted.
  • Instructions for submiting this form can  be found at the bottom of this page.

  • This document is to be completed and submitted by the parent or guardian, not the test-taker.

17-Year-Old Test-taker Eligibility based on the McKinney Homeless Assistance Act

  • This document is to be signed by the applicant in the presence of a Notary Public, notarized, and the scanned PDF must be submitted using the directions below.
  • Faxed copies of these documents will not be accepted.
