School Pre-Closure Form
The following form is to be filled out when a district or individual campus(es) are closing because of severe weather or other disaster. Data from this form will be used to keep the public informed about current school closings, including dates, which campuses have closed, number of students and staff affected, and when school is expected to resume.
The first section has several “Required” fields that must be filled in before submitting the form. The second section, titled “Optional,” is for districts with individual campuses closing – NOT the entire district.
